Accessing your medical details
Your medical details hold information about you and your medical history. Your medical details have your GP record, which includes information like any conditions or allergies you have and any medicine you're taking.
They will also include records from any other NHS service you use, like the hospital, dentist or opticians, and will include letters, test results and appointment notes.
How to get your medical details
You can access your medical details with your NHS number via the NHS App see How to get your medical records page on the NHS website.
If you already have access via SystmOnline click here.
The NHS App gives you access to:
- Book appointments
- Request medication
- See your summary of record
- See your recent results
Enhanced access (over that available on the NHS App) is available. Please discuss your requirements with a medical administrator.
If you are a newly registered patient we will be able to grant access from the date of registration. Once we receive your medical notes from your previous practice we will be able to grant historical access once we have followed our normal procedure for checking accuracy of information.
If you are accessing medical details on behalf of someone else (as a proxy), then you'll need to be nominated as someone they can trust to access them too.
